Our oncology nurse navigators will be with you and your family during your journey. They are available to efficiently coordinate care, assess for barriers and provide individualized resources. These registered nurses are patient-centered and ensure your needs are met.
Your point of contact
Your oncology nurse navigator is your main point of contact when working with your cancer team. They can help you answer questions, provide education and support, and coordinate your care.
Our cancer navigators work with you, your family and your medical team to make sure you receive thorough medical care and support during the entire diagnosis and treatment process. Our navigators provide you with a number of important services, including:
- Helping you talk with your doctor.
- Providing resources to address any barriers that might prevent you from keeping your medical appointments.
- Assisting with referrals required to get the care you need (physical or occupational therapy, home care/hospice, dietary, etc.)
- Helping you schedule appointments for screenings, tests, follow up visits and other care recommended by your doctor.
- Connecting you with community and supportive care services.
- Providing information about the health care services your doctor recommends.
- Connect you with resources to help manage financial and insurance issues (financial counselors also available.)
- Assisting with transportation resources.
