Advance care planning
This process helps you think about your values and goals related to future health care choices, including end-of-life care.
During this process, you select a person who can make choices for you, if you are unable to make them yourself. You can transfer those wishes onto a written plan called an advance directive or Power of Attorney for Health Care.
An advance care directive is important if your attending physician determines you cannot communicate your health care choices because of physical or mental incapacity.
Visit
Marshfield Clinic Health System's advance directive's site for more information, or download the form below. Ask for someone from Social Services to contact you or your family if you have questions.
