Family Health Center has a structured, insurance-like program that can help with health care services at other medical locations.
How does Family Health Center Program work?
We partner with doctors and other providers to deliver health care services through Family Health Center Program. It functions similar to health insurances, but is not considered credible coverage: Enrolled members pay a monthly program fee based on income and family size.
Family Health Center Program can help cover:
- Doctor visits
- Eye exams
- X-rays and lab services
- Dental services
- Substance abuse services
- Case management
- Medications
Family Health Center Program Eligibility
The following are guidelines for membership under Family Health Center Program:
1. Live in service area (see map).
Some counties are in FHC service areas entirely. Other counties are only partially within FHC service areas.

Partial counties:
- Ashland
- Eau Claire
- Marathon
- Sawyer
- Wood

2. Earn a limited income.
You earn limited income if you earn less than two times the federal poverty level.
3. Uninsured or underinsured for health care.
Family Health Center does not discriminate based on age, health condition or citizenship. However, we urge you to apply for Medicare, Medicaid or BadgerCare if you are eligible. Individuals with or without health care coverage are eligible to apply.
