Home Recovery Care

Home Recovery Care is a care option that allows you to safely receive treatment for your medical condition at home without being admitted to the hospital or a skilled nursing facility.

Home Recovery Care is available to patients at the following locations:

Eligible patients who have participated, love it. Home Recovery Care has more than 90% patient satisfaction rate.

Types of Care

Two types of programs are offered through Home Recovery Care: medical and rehabilitation. Your Recovery Care Coordinator will work with you and your doctor to determine which program is right for you.

Medical Care – 30 days

  • Examples of medical conditions include: pneumonia, congestive heart failure and urinary tract infections. 
  • A registered nurse will care for you in your home for 3-5 days, or longer if needed. 
  • You will see and talk to your doctor from a computer tablet.
  • Inpatient and Observation Care (observation care is a shorter episode).

Rehabilitation Care - 60 days

  • Examples of rehabilitation conditions include: infusions, wound care and physical/occupational therapy.
  • A registered nurse, physical or occupational therapist will care for you in your home for 2-3 weeks. 
  • You will see and talk to your doctor from a computer tablet.


What to expect

After you choose Home Recovery Care, you will be able to go home, instead of being admitted to the hospital or a skilled nursing facility. You will have a Recovery Care Coordinator who will be your direct contact for the duration of Home Recovery Care. Your doctors and their staff will oversee your care. 

All the necessary equipment you might need will be set up for you. This includes a computer tablet that allows you to see and talk to your doctor from your home. The computer tablet is provided at no additional cost and must be returned at the end of the Home Recovery Care timeframe. 

Your Recovery Care Coordinator will:

  • Coordinate and communicate your care with your doctor(s).
  • Monitor your vitals daily.
  • Answer your medication questions.
  • Assist in scheduling your follow-up appointments.
  • Provide you with health education.

Your registered nurse will: 

  • Take your vitals, such as blood pressure, temperature and weight. 
  • Administer IV medications.
  • Help you take your medicines. 
  • Assist you with pain management.
